Preparation for hip replacement usually starts with a medical and anaesthetic assessment. The hospital may arrange blood tests, imaging, an ECG, or other investigations depending on age and health. Patients should tell the surgical team about all medicines, supplements, allergies, previous operations, and medical conditions. Some medicines may need adjustment before surgery, but this should only be done under medical guidance.

Improving general fitness before surgery can make early rehabilitation easier. Patients may be given exercises to strengthen the muscles around the hip and may be advised to stop smoking, manage body weight, control diabetes, and treat any active infection. Good nutrition and adequate protein intake can also support healing.

Preparing the home is equally important. Clear walking paths, remove trip hazards, arrange help for the first few days, and place commonly used items where they can be reached without excessive bending. The care team may also explain how to use crutches or a walking frame and what precautions, if any, will be needed after surgery.

It is useful to practise safe transfers before surgery, such as getting into bed, standing from a chair, and using stairs with an aid if these will be needed at home. Patients should also understand which symptoms after discharge require urgent attention and confirm who to contact if there is a wound problem, fever, unusual swelling, or sudden shortness of breath.
